The Birthday sign-up form is the main capture method for growing your Birthday Club. It is enabled by default and will appear on the Order Status Page after a customer has placed an order. It should look something like this:
As we already know the Customer's email address or phone number, we only need to ask the Customer for their date of birth and to then Submit the form.
If a customer has previously signed-up to your Birthday Club, they will not be presented with the Birthday sign-up form after placing subsequent orders.
Date Format
The format of the date presented in the widget is determined by the date format set within the Happy Birthday app. You can change the date format within the settings to suit your requirements.
Previewing the Birthday sign-up form
If you have enabled the Birthday sign-up form within the app settings and would like to check that it is being displayed to customers on your site, you will need to check the Order Status Page from an existing order in your Shopify Admin area.

Form terms text
This customisable piece of text will display underneath the date field within the sign-up form.
Use this field to inform customers what you will be doing with their data.
For example, if you will only be using the customers email/phone number to send the customer a Happy Birthday email/text, something like the following could be used:
By signing up to our Birthday Club, you are consenting to receive a Birthday Email/Text on or around your Birthday each year.
If you intend to send the customer additional marketing material, make sure that you state this clearly within the Forms terms text.
